Ethical Issues Involving Withdrawal Or Withholding Treatments In The Intensive Care Unit (ICU) - Essay Example In terms of finance, withdrawal or withholding treatments in the intensive care unit should defend on the capacity of the relatives of the patient to pay for the costs of health care services. However, financial constraints do not make withdrawing or withholding treatment ethical. Withholding treatment is grounded on medical, ethical, social, and religious values of the patient and the relatives of the terminally ill patient. Different patients have different opinions on the issue. However, rich patients prefer to continue the treatment because they can afford to pay the health care bills. On the other hand, most poor and indigent patients and relatives of poor patients prefer to cut off the life support and other health-prolonging medical equipments and medicines to the patients. Ian Thompson (2006) emphasized applying the utilitarianism ethics concept, the nurse and medical doctor can emphasize that the ethics should focus on the principle â€Å"the end justifies the means†. Under this theory, it would be ethically preferable to remove the life support system if the continued use of the life support system would only prolong the suffering of the terminally ill patient. Under the utilitarian ethics theory, the killing of the patient would be preferable because the end result would be the same: the patient cannot escape impending death. On the other hand, Ian Thompson (2006) also explained deontology ethics states that a rule should be implement in all situations. Under the deontology ethics concept, there are no exceptions to the rule. The rule of avoiding the abandonment or killing of the patient should be to upheld in all situations. Thus, the nurse and the medical doctor should not accept the request of the relatives to pull the plug because they can no longer afford to pay for the services of the medical doctor, nurse, and other healthcare costs. In addition, the medical doctor cannot advise the relatives that it is better to shorten the suffering o f the patient by removing the life support. Further, most medical doctors and nurses implement utilitarianism ethics in their practice. The nurses and medical doctors accept the patients’ family’s request to halt the life support system due to lack of funds to paying the increasing hospital bills of the terminally ill patient. The reason is economics. The healthcare center cannot afford to continue the treatment without payment. Likewise, the patient and the relatives do not have funds to pay for the continued treatment of the patient. In the research conducted, physicians and families of patients on life support system agreed on the decision on when to limit the life support system of their terminally ill relatives (Tschudin, 2003). A research was conducted on 3,498 consecutive patients admitted in six intensive care units. 6.6 percent of the total population had their therapy withheld or withdrawn. Consequently, 221 died in the ICU. The proposal to withhold treatment was advised by the physicians on 210 of the 226 patients. The family members proposed the withholding of treatment in the remaining 16 patients (Esteban et. al., 2001). In another research conducted, physicians in Israel withheld and or had withdrawn the patients’ life support system. This is unethical. The study focused on the intensive care unit of a university hospital in Israel. The findings of the research indicated the life support system in 52 of the 385 patients had cardiopulmonary resuscitation.

The Speckled Band Essay The social and historical settings in the stories mean that although both stories are about crime and detectives, they are almost seperate genres as far as detective stories go. However, they have a very similar structure. Both stories start with a desperate young woman asking for help in solving a crime. In the middle of both stories, the detectives investigate the crimes and find evidence. Holmes does this mostly at the scene of the crime whereas the Op goes to the police station and peoples apartments to interview people involved. Both storied have similar endings in that the detectives solve the crimes very cleverly and then explain to the other characters how they did it. This is a way to explain to the reader how they did it, so the plot makes sense. However, Death on Pine Street ends with a display of the roughness of the Op, as he knocks the tennant down the steps. Holmes did not act in this manner. The differences also lie in the characters other than the detectives. The Speckled Band has far fewer characters, as it is focused on a far smaller location. The victims of the stories are different. In The Speckled Band we feel sorry for Miss Stoner and her sister, they are innocent young girls being cruelly mistreated by a vicious stepfather. We cant really feel very sorry for Gilmore as he is a crook and a serial adulterer. His death may have been an accident, but he probably did deserve it. Dr. Roylott is obviously made out to be the evil, vicious villian of The Speckled Band, but Death on Pine Street is set in an environment where just about everyone is unpleasant and involved in some sort of shady activity; this is again due to the socio-historic setting of the story. The Speckled Band takes on a typically Victorian view of woman, innocently in need of rescue by a heroic character like Holmes. In Death on Pine Street it is just the opposite; the women are tough, they can do things for themselves, and like Cara Kenbrook, are far from innocent. There is also a lot more violence in Death on Pine Street than in The Speckled Band, this again, shows us the difference between 1890s England and 1930s America. The language difference is also clearly seen, Holmes story being spoken in standard english, and Death on Pine Street full of slang terms and americanisms. Both stories give us a good representation of what it was actually like in those periods of time. Because Sherlock Holmes was the first fictional detective it is easy to see how later detective stories like Death on Pine Street were influenced by him. But the whole atmosphere and morality of the two stories is different. The different historical and social circumstances are easily noticable.The language spoken, the attitude of the characters and the overall morals are different. The Speckled Band is a classic, simple Good Versus Evil story, with Good prevailing as usual. However Death on Pine Street has a general feeling of corruption, distrust and seems full of double crossings. The more modern story is influenced by classic features of Holmes such as his strength of character, brilliant mind and use of a complex mystery, however the modern American setting has meant changes to the classic English style. Death on Pine Street blurs the borders between good and evil, which I find interesting. The classic Holmes approach, whilst enjoyable, can seem unrealistic, considering present day life. Both stories are very enjoyable in their own way, as I stated before they could almost be in seperate genres. The element of mystery is used well in both stories, and both stories have interesting characters. I think the fact that they are both so ingenious is what compells us to read on; we want to know how they managed to solve such a complex puzzle. Although from differing times and places, they are both prime examples of what a detective story should be.

When most people hear the name Joseph Stalin, they usually associate the name with a man who was part of the Communist Party of the Soviet Union and was responsible for the deaths of millions of people. He was willingly to do anything to improve the power of the Soviet Union’s economy and military, even if it meant executing tens of millions of innocent people (Frankforter, A. Daniel., and W. M. Spellman 655). In chapter three of Sheila Fitzpatrick’s book, Everyday Stalinism, she argues that since citizens believed the propaganda of â€Å"a radiant future† (67), they were able to be manipulated by the Party in the transformation of the Soviet Union. This allowed the Soviet government to expand its power, which ultimately was very disastrous for the people. The Soviet citizens during the 1930s, particularly the younger ones, believed â€Å"they were participants in a history process of transformation, their enthusiasm for what was called ‘the building of socialism’† (68). The Soviets built hotels, palaces, and had blueprints displayed all throughout â€Å"that was supposed to set a pattern for urban planning throughout the country and provide a model of the socialist capital for foreigners† (69). To further transform the Soviet Union, state officials encouraged citizens to help improve the literacy rate and recognize the many heroes of the socialist state. These heroes, including Joseph Stalin, â€Å"received huge amounts of fan mail and were lionized on appearances throughout the country† (72). They also encouraged the remaking of individuals, particularly through work. Before the transformation, many did not enjoy working, but â€Å"under socialism, it was the thing that filled life with meaning† (75). Numerous interviews an author had with â€Å"transformed† felons, illustrated that even criminals could be transformed into good citizens through work (76). However, Sheila Fitzpatrick argues that these interviews were â€Å"clearly a propaganda project.† The transformation of individuals also included citizens’ desire to become more cultured. Many Soviet citizens characterized peasants, those who were not yet part of the transformation, as â€Å"economical[ly] and cultural[y] backwards† (70); thus, the people wanted to be more cultured to distinguish themselves from the lower-class. Such things as brushing teeth, table manners, and public behavior allowed them to be distinguished (80). Fitzpatrick says on page 80, â€Å"Newspapers and journals carried regular accounts of successes in mastering the first level of culture, [†¦] these should not always be taken literally.† Fitzpatrick again emphasizes that much of the reports during this period were propaganda.

The Quintessential Rebel In Allan Sillitoe’s The Loneliness of a Long Distance Runner, we are introduced to Smith, a man with his own standards, beliefs, values, and battles. As we are taken through the story of a period of his live, we come to understand what Smith really stands for. He is a diehard rebel that is destined to always stick to his beliefs, and is willing to sacrifice all in a battle against his greatest enemy and opressor, society. He willingly shares his thoughts with the reader, and often times his thoughts develop as he is telling his story giving us an up-close look at the inner workings of Smith’s mind and personality. Smith belongs to a group of people he calls the Out-Laws. It is the underprivileged lower class poor street criminals. Crime runs in Smith’s family, and being born into poverty he nether sees, nor is even willing to contemplate a life without crime. At a point he hints on having some communist views, and perhaps suggests that his father had communist friends, if he wasn’t one himself. Fatally inflicted by cancer, Smith’s father died a painful death. We later find out that it was Smith who found his father breathless in a pool of his own blood, and to this day has a great deal of respect for him. The first time Smith’s family gets a taste of a financially comfortable life is when the factor y his father worked in gave them a lump of cash upon his father’s death. â€Å"†¦a wad of crisp blue-back fivers ain’t a sight of good† (Sillitoe, 20) says Smith as the one break his family got was only due to his father’s death. Smith is not money hungry, he steels simply to get by. Johnson Johnson and its subsidiaries have approximately 115,600 employees worldwide. Johnson Johnsons primary focus has been on products related to human health and well-being. According to MarketWatch (2006) states that Johnson Johnsons worldwide business is divided into three segments: Consumer, Pharmaceutical and Medical Devices and Diagnostics. The Consumer segment manufactures and markets a range of products used in the baby and child care, skin care, oral and wound care and womens health care fields, as well as nutritional and over-the-counter pharmaceutical products. Finally, the studys findings were that the company had a diversion problem which involves the selling of a legitimate product in markets other than  those for which it was intended, and it opens the door for counterfeit products to enter the supply chain (Wald Holleran, 2007, p. 58). One example was that no single person or group at the MDD level was responsible for protecting MDDs brands or its products. Firstly, the team developed a brand protection policy that is now being used as a guideline by MDD. The policy addresses the protection of intellectual property, accountability issues, product protection and supply chain standards, and incident reporting and enforcement protocols. Secondly, an organization was put in place throughout MDD to address brand protection across the business. The team helped to define the key resources and assign them specific roles, responsibilities and accountability. These roles include a vice president and executives responsible for product protection, supply chain management, and incident reporting and enforcement. Thirdly, for those products at highest risk, the company will develop product protection plans that include overt and covert anti-counterfeiting  technologies and features based on their risk levels. This allows the company to consistently manage its products throughout the organization, from sourcing through distribution. The France declares them as the secular, social, individual and democratic republic. The government of France divides into a legislative, a judicial branch and an executive. The prime minister shares the executive powers with the president. Parliament consists of the Senate and the national assembly. It passes vote and statutes on the budget and it also controls the works of the executive through questioning in the parliament house and by organizing enquiry commission. The members of the constitutional council are appointed by the president of national assembly, the president of the senate and the president of the republic. The judiciary system of the France is based on the civil law system which was evolved from Napoleonic codes. The judiciary system is divided into the administrative code and the judicial branch. They have their own supreme court of appeal like Conseil d à ©tat for the administrative purpose and Court of Cassation for judicial purpose. The French government also has various bodies to check whether the powers are abused by anyone or not. France was also the founding member of the European Union. The third republic was formed after the Prussian Franco wa r in 1870 and lasted till the defeat of the military in year 1940. During the World War I the great loss of material and troops occurred. France government formed the huge border defenses known as the Maginot Line and alliances to defeat the German strength in the year 1920. Although France was defeated early in the world war III then also they gain the power again in June 1940. In July 1940 France was divided into two sections. One section was ruled by Germens and another was controlled by the Vichy France. But in year 1942, the Italian and German force conquered the France including Vichy region also and formed the allied forces. In 1944, Allied force liberated France after four year of occupation and services. After the world war II France faces new problems. Initially the provisional government was led by Gen. Charles de Gaulle but after the short period of time the fourth republic was formed by the new constitution. It was parliamentary form of government which was controlled b y series of coalitions. The lack of agreement between the French military and the members of the constitution was the main cause of the change of the government. On May 13, 1958 finally the government structure collapsed due to the tremendous pressure which was generated by the four years war with Algeria. In June 1958, fifth republic was formed and General de Gaulle became the prime minister and in December of the same year he was elected as a president. After the seven year in the 20th century, the people of France for the first time got the liberty to elect their president through ballot system. De Gaulle won the election after defeating Francois Mitterrand with 55% share of vote. After elected as a president of the country De Gaulle conducted a referendum which was concern on the creation of twenty one with the less political powers. But this proposal was rejected by the members of the government after that he resigned from his post. After him, Gaullist Georges Pompidou was elec ted as president for year 1969 to 1974 then Valery Giscard dEstaing for year 1974 to 1981. After that Francois Mitterrand was elected for year 1981 to 1995 and then neo-Gaullist Jacques Chirac for year 1995 to 2007. Nicolas Sarkozy was elected in May 2007 as sixth president of France under the fifth republican. It remarks as increasing of social and economic reforms. During the five year term of Sarkozy, he faced tremendous pressure to improve the economy, employment rate and reduce the sizable budget deficit of the government. He has also reintegrated the north Atlantic treaty organization with France. According to this constitution the president was elected for the seven year term but in 2002 the presidential period of work got reduced to five years. In year 2008, the new constitutional reform was passed again which limits the presidential period to two consecutive terms. The next legislative and presidential elections are scheduled in year 2012. The executive branch of France is consisting of the prime minister, the president, the bureaucracies of many ministries and the government. The president has the right to elect the prime minister, rules over the cabinet and gives the instruction to the armed forces. The president also has right to submit the question to the referendum. At the time of emergency the president has rule by decree and dictatorial powers but with permission of the parliament. The head of the government is prime minister and his cabinet is composed of minister delegates, secretaries of state and number of ministers. Under the fifth republic, president leaves the day to day policy decision to the prime minister and its government. Every year the members of parliament meet once for the nine month session. Under the special situation the president has special right to call the additional session. Even the legislative branch has few executive powers then also the national assembly has power to remove the existing government if the votes of the members are in majority. The parliament of France consists of a Senate and a National Assembly. The principle legislative body of France is national assembly. The members of national assembly are elected for the five year through voting. The Electoral College has the right to choose the senators. In 2003 a new rule is passed the senators will serve for six years but one half of the members will be renewed after three years. The legislative powers of the senators are limited as compared to the national assembly. Initially the government has power to solve the agenda of parliament but after the constitutional reform which was passed in 2008 parliament got the right to solve their own agenda. In July 2008, a new constitutional reform was passed which limited the process to one bill in each session, finance on the social security and to the vote for the national budget. Impact assessment is necessary from the September 2009, for all the draft laws which is going to the parliament and the council state. The best feature of the judicial system of the French is that the council of state protects the basic rights when it is violated by the action of the state and constitutional council protects the basic laws when it is violated by the new laws. The function of the constitutional council is different from the council of state and they provide the justice to the citizens who have claimed against the administration. There are various ordinary courts such as the criminal courts, the police courts, the industrial courts and the commercial courts which settle the disputes which arise between the citizens and also between the corporation and the citizens. The cases which were judged by the ordinary court can be reviewed by the court of appeals. Political system On May 16, 2007 Nicolas Sarkozy joined the office under the Fifth republic as the sixth president. On April 22, 2007 during the first round of election, Nicolas Sarkozy placed first, the head of the Union for popular movement party. The Segolene Royal, the candidate of socialist, placed second. Francois Bayrou of centrist placed third and Jean Marie Le pen of extremist was placed fourth. In the second round of the election on May 6, 2007, Sarkozy defeated Segolene Royal by 53.06%. The defeat of Royal in the presidential election was the third largest defeat of the any Socialist candidate. The focal point of the campaign of the Sarkozys party was to implement the various reform like economic and market oriented reforms. After electing as the president he first gave the green signal to implement these reforms and worked on the European Union treaty which was rejected in the year 2005. I am taking some classes that will eventually qualify me to major in Astro – Physics, or Chemical engineering, I also want to work with NASA and train as an astronaut. It was amazing to know that Dr. Mae C. Jemison who happens to be the youngest of three children born to a middle class African American family, Charlie Jemison, a maintenance worker and his wife, Dorothy, a teacher. Dr. Mae C. Jemison was the first black woman astronaut to be in space in an era filled with segregation and racism, she is a Chemical engineer, scientist, physician, teacher and astronaut, she has a wide range of experience in technology, engineering, and medical research. In addition to her extensive background in science, she is well-versed in African and African-American Studies, speaks fluent Russian, Japanese, and Swahili, as well as English and is trained in dance and choreography. Dr. Mae C. Jamison was an inspiration to me, and probably to many African American women. She was full of resilience and determination especially to have reached and achieved success in an unusual field of endeavor for many African American women, I applaud her determination to make a difference among the African American women and blacks in Diaspora. After graduating from Morgan Park High School in 1973 at the age of 16, Dr. Mae Jemison earned a BS in Chemical Engineering from Stanford University, while also fulfilling the requirements for a BA in African-American Studies. After earning these degrees in 1977, she attended Cornell University and received a Doctor of Medicine degree in 1981. During medical school she traveled to Cuba, Kenya and Thailand, providing primary medical care to people living there. This is an indication of her humanitarian efforts and eagerness to reach out to the less privileged population. Having a desire to do more with her life, she enrolled in graduate classes in engineering and applied to NASA for admission to the astronaut program. She was turned down on her first application, maybe because she is a black woman, but she persevered and in 1987 was accepted on her second application. She became one of the fifteen candidates accepted from over 2,000 applicants. When Dr. Mae Jemison successfully completed her astronaut training program in August 1988, she became the fifth black astronaut and the first black female astronaut in NASA history.
